And the answer is:

Piano tuner's voicing pliers, used to pierce the felt hammers to improve the tone. After a lot of use the felt gets compressed and doesn't sound like it should, if the hammers aren't properly maintained you could end up with something that sounds like a tack piano.

well, like i always say, "you don't have to know all the answers. just know where to go to find them."
my sister is a piano technician.
i emailed this thread to her.
she is familiar with the tool shown, but admits that she would never use one. she deals with high-end customers and their pianos. that tool, she says, is at best, a stop-gap measure, to fix a piano that really needs new hammers.
